For medically necessary clinical diagnostic neuropsychological evaluations, we are "in network" providers for Medicare, and several major commercial health insurance carriers including Anthem-Blue Cross Blue Shield, Aetna, Humana, and United Healthcare. We require a referral from a patient's physician to use health insurance coverage for our evaluation services. We will contact your health insurance company to obtain an estimate of what your out-of-pocket costs will be prior to the evaluation.
We are Medicare providers and are happy to accept original Medicare coverage. However, we do not accept any Medicare Advantage insurance plans.
Aviation evaluations for the FAA, employment-related evaluations, and evaluations for forensic, legal, school/education, and other administrative matters are not considered medically necessary and are not covered by health insurance. If you are interested in these services, please contact our office to discuss our typical fees. Payment is due at the time of service.
